Dr. Carlos Perez

Dr. Carlos Perez received his undergraduate degree in 1984 from the University of Louisville. He then went on to pursue his Doctor of Dental Medicine degree in 1988. He is currently a member of the Louisville Dental Society, Kentucky Dental Association, and the American Dental Association, as well as the Southern Indiana Dental Association. He has been practicing dentistry for over 30 years and is dedicated to providing high-quality dental care to all of his patients and the community. He is bilingual and speaks fluently in English and Spanish languages.

Dr. Perez has been married to his wife Charla for 40 years. They have two adult children Alex and Nick, and have 3 beautiful granddaughters. On days not spent at work, you can find him hunting and fishing in the countryside of beautiful Southern Indiana. They also take delight in dining out at various local restaurants. He and his wife are actively involved in their church home and spend time there as well. His leisure time also includes engaging with his beloved black lab, Duncan.

 

Dr. German Bazan

Dr. German Bazan was born in Cuba and grew up in Louisville. He received his bachelor’s in Biology and Doctor of Dental Medicine degrees from the University of Louisville. Having graduated in 2018, he practiced in Cincinnati for 5 years, providing dental care to the underserved population. Dr. Bazan loves going to work every day educating his patients about their dental health & helping them attain their best smile possible. His areas of expertise include general dentistry, routine family care, crowns & bridges, removable partials and dentures, tooth extraction, and emergency services. He regularly attends continuing education conferences to stay current on all the latest dental materials & techniques.

Outside the office, Dr. Bazan enjoys spending time with his wife, family, and friends. His hobbies include watching college sports, traveling, and boating. His family is excited to return to Louisville and raise their soon-to-be-born baby boy!
